We need a completed form for every student (please select the correct form).
ALL students need certain things outlined on their form and some items

are “Optional”. Note - Payment Plans are available see below

Payment Plan Schedule: 1st Payment/Deposit ($100) at Registration 4/29/25

2nd Payment ($200) due by 6/1/2024

3rd Payment ($200) due by 7/5/2024
Final Payment/Remaining Balance due by 8/2/2024

*Important Payment Info: Fees can be paid in full at any time.
